Liposuction- Why Suffer for Beauty?

Wednesday, March 24th, 2010

Traditional Liposuction is suffering and so are the patients who are going under the knife for this procedure! Many of us desire leaner, more sculpted, figures and have found that liposuction may be the answer to our prayers. Finally, a way to get rid of unwanted fat that doesn’t respond to diet and exercise! But with so many different choices of “liposuction” procedures out there, Smartlipo, Body Jet Lipo, VASER/ultrasound Lipo, or traditional power-assisted or mechanical liposuction…..how do you choose which is the right one for you?

Well, let me explain - Traditional Liposuction, whether power-assisted or hand-driven, employs larger cannulas to mechanically disrupt (think - cheese grater) the fatty, subcutaneous, layer followed by use of a suction device to remove the chunks of fat. Due to the brutal nature of this procedure, most surgeons opt to treat patients under general anesthesia, but some do perform the operation under tumescent (local) anesthesia with sedation. The mechanical technique does not give the physician the ability to avoid nerve endings or blood vessels and, therefore, results in a great deal of bruising, swelling, and superficial nerve damage. In addition, when patients are treated under general anesthesia, not only do this increase the risk of the procedure due to the use of the anesthetic medications, but also takes away the verbal feedback from the patient as to their level of discomfort - a vital tool when trying to avoid complications, like perforating bowel or other internal structures. Compression garments must be worn for 4-6 weeks in effort to decrease swelling and help with skin retraction post-operatively. The cost for these procedures is typically higher due to the need for general anesthesia and use of an operating room.

So, you still want to get rid of that fat?! OK, there ARE other options. Laser-assisted liposuction came on the scene about 10 years ago. These devices use small, laser cannulas (usually about 1 mm in diameter), inserted under the skin after infusion of local anesthesia, to gradually “melt” away the fat in the area before the fat is then suctioned. This tool has also been shown to produce approximately 50% more skin tightening due to the effect of the laser energy on the overlying skin. I have found this to be a wonderful tool to address small body areas or localized fat deposits. Results are seen quickly because the laser spares the surrounding vasculature and nerve endings from damage, so we see less bruising and swelling. Compression garments are only required for 1-2 weeks after the procedure which is typically done on an out-patient basis in the office. This is a great option for treatment of superficial fat in relatively small, thin, individuals. At our office, we use the SmartLipo laser which was the first FDA approved devise for this purpose and has stood the test of time compared to it’s competitors on the market. Price for treatment? Approximately $1500 per body area.

Have more than just a “little” to lose? Newer technologies include ultrasound-assisted liposuction, such as the VASER device and water-assisted devices such as the Body-Jet. We use the VASER here in our office and I love it. This tool allows me to treat larger patinets, patients who may have had previous liposuction procedures (who may have some scarring or fibrosis in the area), and men, who, typically, have more fibrous fat. These patinents need a more aggressive approach. The powerful ultrasound energy targets the deeper fatty layers and easily disrupts tougher fat, such as male breast tissue or fat that has been previously liposuctioned. Patients get more dramatic results and seem to be happier with their overall treatment. Post-procedure bruising and swellling is expected for up to two weeks after the procedure but much better tolerated than what is typically seen after traditional liposuction cases. Body Jet Lipo is new to the market and employs high-pressured water to disrupt the fat cells before suction is applied. The makers of this device tout less downtime and discomfort for their patients with this method. Having seen the procedure performed first-hand, I am not convinced that it offers any real benefits over the ultrasound device, except that it may play a better role in fat harvesting for reinjection into other areas such as the breasts or the face, since it does leave the fat cells intact rather than melting them before suction. Both of these techniques are good at targeting deeper fat and may be used in larger individuals. That being said, I would still recommend use of a laser device to help aid in skin tightening as an adjunct to either therapy. In my practice, I routinely use the VASER device to get at the deeper fat layer followed by the SmartLipo to target the superficial layer and give the finishing touches. Price? Approximately $2000 per body area. Results? Perfection!

Lasers Resurfaced

Monday, March 1st, 2010

Laser skin care has come a long way. In the “old days” (1980’s-1990’s) we routinely preformed laser resurfacing treatments for wrinkles and skin laxity with ablative CO2 lasers. These treatments were extremely painful and therefore required general anesthesia or deep sedation and resulted in months of downtime due to the extended healing process. That being said, the results were wonderful. We could achieve amazing transformations with only 1 treatment, albeit the patient didn’t want to leave the house to show off their fresh new face for 3-6 months! So the pendulum swang in the opposite direction - toward non-ablative, “fractional”, laser treatments that work to remodel collagen from the inside-out. These treatments allow patients to get smoother, tighter skin with less pain (only topical numbing agents required) and less downtime (typically 2-4 days of redness). The trade-off here is that most patients require 4-6 treatments spaced 6 weeks apart to achieve adequate results and for patients over the age of 50, less predictable results. So in walks the latest in laser technology….the Fractional CO2 Laser! Finally, a laser that offers an ablative option (aggressive, impressive results) combined with fractional technology (less downtime). These lasers typically yield maximum results in only 1-3 treatments with as little as 4 days downtime. At last! An answer for those of us suffering from skin laxity, wrinkles, and discoloration!

Paradise Medspa and Wellness proudly offers both the non-ablative fractional laser treatment as well as the latest in Fractional Co2 technology! For younger patients looking to improve the quality of their skin texture, treat mild-to-moderate wrinkles, skin laxity or acne scars or for those who can’t afford to take any time out of their schedule for the healing process, we recommend a series of Affirm Multiplex Laser treatments. For patients over the age of 50 who may have more significant sun damge or skin laxity, we recommend the new SmartSkin CO2 Laser. Two great options giving us the versitility to meet the skin care needs of any patient.

Become Beach Body Ready

Wednesday, June 24th, 2009

By Dr. Rebecca Weiss

With the summer months ahead of us, now is a great time to get yourself “bikini ready” with body contouring services. More and more patients at our office are specifically requesting the VASER Liposelection procedure. Despite its name, VASER does not utilize laser energy, but rather, uses powerful ultrasound energy to melt fat. As opposed to laser body contouring services that are best for patients with smaller, more isolated areas of fatty tissue, the VASER procedure is perfect for patients with more pronounced areas of fat. I also find it better for my male patients who tend to have more fibrous fat.  Results with the VASER procedure can be seen immediately with very little downtime due to the fact that the devise coagulates blood vessels in the treated areas thus resulting in less bruising and swelling than seen with traditional liposuction. VASER is preformed while patients are awake, without the need for general anesthesia.  Ideal candidates for this procedure are patients in good health who exercise regularly but still have stubborn areas of fat that do not respond to diet and exercise. For more information, you can visit their website at www.vaser.com!

Facials are not Just Pampering

Monday, June 15th, 2009

by Meredith Morgan

Facials are not just for pampering; they are a great way to maintain good skin, or to work towards a common goal. For example, I see many clients who suffer from acne, especially in adults. Most of them want the quick fix; one facial or perhaps chemical peel, and poof, their acne is gone.  I wish this was the case, but unfortunately it takes more effort on their part, as well as mine as an aesthetician.  If someone comes to me with a skin concern, I always suggest the facial club. ($75/ mo, $25 extra for upgrade) This insures the client will come in once a month for a medical grade 75 minute facial, with the option of upgrading to a chemical peel, or microdermabrasion.  Each month I will evaluate their skin, note the progress, and I will customize the facial based on what their skin needs. With monthly facials, good skin care products, and the doctor’s advice we can achieve desired results.  If one wants to see changes in their skin, I suggest they make a commitment, and I too will make the commitment to help them achieve their goal. It truly is another great day in Paradise when I see positive changes in ones skin!

Your lashes go on for miles!

Monday, June 1st, 2009

by Janet Fales

The latest buzz around Paradise is about a new product called Latisse. I always see girls or women trying to make their lashes longer and fuller with mascara but if you don’t get it just right or glop it on it always looks like you have tarantula legs for eyelashes. It really drives me crazy to see beautiful women of all ages with clumpy lashes. That is the best part about Latisse, it really makes your eyelashes longer, fuller, and darker. So you either don’t need to wear mascara nearly as often because you have fabulous lashes normally or if you want to glam them up for a night out your lashes are already longer so you don’t need as much mascara to begin with! Plus you save mascara - that’s a few bucks in your pocket right?

You are probably are thinking this too good to be true, but Latisse really works! Check out the before and after pictures on the Latisse website.

The best part about Latisse is that it easy to use and you get great results; also it is the only thing on the market of its kind that’s actually FDA approved.

April is Rosacea Awareness Month!

Wednesday, April 1st, 2009

by Dr. Rebecca Weiss

Did you know that Rosacea is often misdiagnosed as “adult acne”? Rosacea sufferers may have similar findings to acne patients in that they will have facial redness and pimples, however, their skin will respond differently to treatment. Rosacea patients have more skin sensitivity and worsening symptoms with triggers such as sun and wind exposure, spicy foods, alcohol, and stress. If you think that you may have Rosacea, do not fear! There are many effective treatments available, including a new skin care system by Obagi, called Rosaclear. This system is the first and only prescription-strength system to combat all the symptoms of Rosacea. Other treatments include LED light therapy, photofacial treatments and prescription medications. With the appropriate combination of treatments, Rosacea can be kept under control so that patients can have a clearer, calmer complexion that they desire.
